Pennsylvania requires a qualified supervisor to submit a notarized affidavit or unsworn declaration before beginning and annually by August 1, maintain a portfolio and instruction-time/subject records, complete standardized testing in grades 3, 5, and 8, and submit a qualified evaluator’s annual report to the district by June 30.

What Is Required
Pennsylvania requires a qualified supervisor to submit a notarized affidavit or unsworn declaration before beginning and annually by August 1, maintain a portfolio and instruction-time/subject records, complete standardized testing in grades 3, 5, and 8, and submit a qualified evaluator’s annual report to the district by June 30. Elementary programs provide 180 days or 900 hours. The companion contains the declaration, objectives/subject crosswalk, calendar, portfolio, test, medical-record checklist, and evaluator packet.
Tier 3 State Companion: Editable, parent-reviewable 36-week Curriculum Summary Draft with student and grade/year details, parent and instructor fields, four nine-week review/report dates, and subject-by-subject scope, sequence, objectives, resources, methods, and progress evidence. It also adds affidavit/unsworn declaration, annual objectives, 180-day or 900-hour elementary calendar, portfolio, grade 3/5/8 tests, and June 30 evaluator report. Parent review and submission remain required.

What Is Not Required
